Make a Difference Within Our Community
Currently, the Mississaugas of Scugog Island First Nation (MSIFN) is looking for an experienced Community Health Representative to join our community and work with our members and management team in Scugog, ON on a fixed term contract. The hours of this role are Monday to Thursday, 8:30 am to 4:30 pm, and Friday 8.30 am to 1:00 pm. The Healthy Lifestyle Coordinator is required to be onsite Monday to Friday.
As our new Community Health Representative, you will report to the Health and Social Services Manager and will be responsible for providing assistance and support to the entire health team and acts as a liaison to the community.
Your typical responsibilities include:
The Community Health Representative (CHR) is responsible for health promotion, prevention and health education and activities for the Mississaugas of Scugog Island First Nation. The CHR promotes and motivates MSIFN community to participate in health program opportunities, contribute to improved community health conditions and assume active responsibility for improving and maintaining optimum level of health.
Key activities:
- Provide and participate in the delivery of community health programs through clinics, workshops and activities
- Encourage community to participate and attend education and health promotion sessions
- Plan and arrange specialty clinics with physician specialists or other health care providers
- Consult and collaborate with other colleagues of the team to ensure client care is appropriate and is directed towards the clients’ goals
- In collaboration with health care providers, facilitate individual learning to members and families in relation to patient illness or injury as directed
- Act as a resource for other community-based health care workers
- Promote a safe and healthy environment throughout the community
- Advocate for individual and community programs dealing with health-related issues
- Develop new programs, initiatives and assist with proposal development to provide community health information, instruction and guidance to MSIFN community
- Educate the community in safe health practices and proper utilization of health care systems, assist community to identify their needs and effective community services
- Promote and provide MSIFN community members with information on various health services and providers
- Advises families and individuals of health facilities and related social benefits available from the Non-Insured Health Benefits (NIHB), and other federal, provincial government agencies and community sources
- Organize and implement preventative health care gatherings, workshops and information sessions
- Adhere to Health Care standards, policies and procedures
- Participate in the planning, development and evaluation of programming
- Report potential or actual unsafe situations for clients to appropriate team members
PAY RATE: $28.33/hr (I1)
MINIMUM QUALIFICATIONS:
- Certificate in a health-related field
- Minimum one (1) years of related work experience and minimum three (3) years of total work experience or an equivalent combination of education, training and experience as deemed by the hiring panel
- Preferred work experience with a First Nation or Indigenous Organization
Other related skills required:
- Valid driver’s license and $1M liability insurance
- CPIC/VSS acceptable to position upon conditional offer
- Current CPR and First Aid
- Strong written and verbal communication skills
- Excellent interpersonal skills when working with colleagues, community and external partners
- Ability to work with confidential and sensitive information
- Detail oriented and excellent organizational skills
- Ability to assume responsibility and meet deadlines
- Ability to analyze problems, recommend and implement solutions
- Initiative and ability to work independently as well as a part of a multi-disciplinary team
- Flexible to work varied shifts as required
- Knowledge and understanding of Indigenous culture, traditions, teachings, community dynamics
- Knowledge of legislation governing First Nations
- Knowledge of concepts, principles and practices of community health
- Strong working knowledge of Microsoft Office software suite
Considered an asset:
- Experience in community health is an asset
- Post-Secondary diploma or degree is considered an asset
